Cytoreductive Surgery for Ovarian Cancer Treatment

What Is Cytoreductive Surgery?

Due to its subtle symptoms and lack of early detection, patients with ovarian cancer are often diagnosed at an advanced stage when the disease has already spread throughout the peritoneal cavity and other abdominal organs. Cytoreductive surgery is done to reduce this tumour burden to a minimal level, known as optimal cytoreduction.

The Role of Cytoreductive Surgery

Cytoreductive surgery is a complex and extensive surgery that requires meticulous planning by the surgical team.

  1. Initial Staging and Diagnosis
    CRS also serves as a diagnostic tool and during this, the surgeon can assess the extent of the disease, determining its stage and collecting tissue samples for biopsy.
  2. Debulking of Tumour
    Cytoreduction is primarily done to physically remove as much of the visible tumour as possible. This includes the removal of the ovaries, fallopian tubes, uterus (total hysterectomy with bilateral salpingo-oophorectomy) and potentially other organs or tissues that may be affected by the cancer.
  3. Improve Chemotherapy Response
    By removing visible tumours, CRS allows chemotherapy to be significantly more effective. Microscopic residual cancer is more susceptible to the cytotoxic effects of chemotherapy drugs.

In some cases, CRS can be combined with Hyperthermic Intraperitoneal Chemotherapy (HIPEC), where the therapy is delivered directly into the abdominal cavity to kill any remaining cancer cells.
